TOR: Madatory Health Forms

NOTE: The form links have been fixed!

All individuals, adults and children, traveling in the official Broughton party to TOR will need to complete all applicable attached forms and submit on Sunday, Dec. 16. This means parents, aunts uncles, grandparents, etc. etc. MUST have this form completed prior to departing for Pasadena. Please remember that all people traveling to TOR will need to receive their Flu vaccination PRIOR to leaving. In order for this vaccination to be most effective, you should receive it no later than 2 weeks before we leave for Pasadena.

There are three forms. Please read the following instructions for each form and complete ALL that are applicable for your situation.

First form: Broughton Band Health HX Update

This form is for ALL current Broughton High School Band Students and will need to be completed for ALL band students. This form requests any information that has changed since you submitted your health history information in the Fall as well as the date you received your Flu shot. Please remember that each band student is required to have their flu shot prior to leaving for Pasadena. If you are not able to receive the shot due to medical reasons, please enclose documentation from your healthcare provider.

Second Form: Adult Health HX Form

All adults traveling in the official party need to complete this document. This includes the college students that are returning to march in the parade. Since most of them are 18 or older, we need to capture their information as an adult participant. Please remember that everyone traveling in the official TOR Broughton party is required to have their Flu vaccination prior to leaving for Pasadena. If you are not able to receive the shot due to medical reasons, please enclose documentation from your healthcare provider.

Third Form: Sibling Medical History Form

This form is to be completed for all children traveling in the official party. This includes brothers, sisters, cousins, etc. even if they are not marching in the parade. Please remember that everyone traveling in the official TOR Broughton party is required to have their Flu vaccination prior to leaving for Pasadena. If you are not able to receive the shot due to medical reasons, please enclose documentation from your healthcare provider.

THESE FORMS ARE NOT TO BE TURNED IN TO JR. PLEASE BRING THESE FORMS TO THE MANDATORY MEETING ON December 16th. THERE WILL BE A TABLE SET UP IN THE LOBBY TO COLLECT AND RECORD FORMS RECEIVED. ALL FORMS MUST BE RECEIVED ON December 16th.

These forms are designed to ensure that the medical team has all of the necessary information related to you and your health during this trip. We are traveling with over 300 people so please take the time to complete appropriately and in a timely manner.
